Online Registration Guide

Welcome! Please complete the following online registration process.

- Visit http://www.ghsd.us/ and click on the Online Registration link located under the Parents and Students tab.

- If you have never completed an online registration using this system then you will need to create an account.  This will allow you to securely save your work and come back at a later time if necessary.

- If you already have an account, you can sign in and complete the form. (You should use the same account to complete forms for multiple children.)

- Be sure to click “Submit” on the top right corner, this will send all the information you have entered to the school.

The following documentation is required to complete the enrollment registration process. You may upload the documentation through the online registration system, fax (209-745-3936), or hand deliver to Galt High School (145 N. Lincoln Way).

o Birth Certificate (Proof of age)

o Immunization Record

o Two forms of proof of residency (only two of the following are required)

                    -current utility bill

                    -a deed, or rental agreement

                    -current car registration or insurance

                    -any type of correspondence from a local or state agency

                    -recent pay stub that shows parent’s/guardian’s address

o Parent/Guardian photo ID

Other (if applicable):

o  Transcript (official or unofficial) and/or withdrawal form

o  Custody Agreements

o  Affidavit of residency

o  Caregiver’s affidavit

o  IEP/ 504 plan

o  Inter/Intra- district agreement

o Other:

¡Bienvenido! Por favor, complete el siguiente proceso de registro en línea.

- Visite http://www.ghsd.us/ y haga clic en el enlace de Registro en línea ubicado en la pestaña Padres y estudiantes.

- Si nunca ha completado un registro en línea usando este sistema, entonces necesitará crear una cuenta. Esto le permitirá guardar de forma segura su trabajo y volver en un momento posterior, si es necesario.

- Si ya tiene una cuenta, puede iniciar sesión y completar el formulario. (Debe usar la misma cuenta para completar formularios para varios niños).

- Asegúrese de hacer clic en "Enviar", esto enviará toda la información que ha ingresado a la escuela.

Se requiere la siguiente documentación para completar el proceso de registro de inscripción.  Puede cargar la documentación a través del sistema de registro en línea, enviar un

fax (209-745-3936) o entregarlo en la oficina de consejería de la escuela (145 N. Lincoln Way).

o Acta de Nacimiento (comprobante de edad)

o Comprobante de Vacunas

o 2 verificaciones de Residencia Actual (2 se requiere de las siguientes facturas de servicio)

                    - factura de servicios actual

                    -documento de pago de la casa o contrato de alquilar (de renta)

                    - actual registro de automóvil o seguro

                    - cualquier tipo de correspondencia de una agencia local o estatal

                    - talón de pago reciente que muestra el domicilio del padre/tutor

o ID de foto de padre/tutor
